Waivers and Forms
Print, sign, and send us the Camp Tilikum Participation Agreement. You can also bring the printed and signed copy with you when you arrive at camp.

If you are under the age of 18:
You will need a parent or legal guardian to complete your registration, and you must bring your printed and signed forms and activity waivers when you arrive on site.
